How To Create A Link To Your Product Request Form

  1. From your dashboard, select Forms.
  2. Select the Request tab
  3. Click the "+ Create" button
  4. Name your form
  5. Edit the title (optional)
  6. Edit the subtitle (optional)
  7. Ensure the Active toggle is On
  8. Ensure the Show in consumer app is On
  9. Optional: If you'd like each question on the form to be presented to the customer sequentially, toggle the Sequential toggle ON. If you'd like them to be presented on a single page, leave the toggle OFF
  10. If you'd like customers to only be able to submit the form one time, toggle Prevent retaking ON.
    1. For request forms, we recommend leaving the toggle of to allow customers to request your products at multiple stores or in the event your brand launches new SKUs.
  11. Click Submit.
    1. If you'd like to add additional questions to the request form, click Edit
      1. Click Save when you'd like to preserve changes
      2. To add text to the "Ending" page after a customer submits their product requests, click Ending.
      3. Add any text, links, or promo codes you'd like in the text box.
      4. Click "Save" to preserve changes
  12. From the Form > "Your Form's Title" screen, open the Tracking Links section.
  13. Click the "+ Create link" button.
  14. Name the link.
  15. Leave the Active toggle On
  16. Toggle UTM parameters on to add UTM parameters you'd like to use to track the source and medium of where customers engage with the request form
  17. Toggle Generate a QR code ON if you'd like to create a QR code for the form to share at in-person events and on printed materials.
  18. Click "Submit"
  19. Your link will be listed in the Tracking Links section
  20. Copy the URL as written on the page.
    1. To ensure the click through and engagement data is tracked accurately, only share the URL as written on the page. Do not copy the URL that the link redirects to when clicked.
  21. Share the link wherever you engage with your customers:
    1. Via email
    2. On your website
    3. On your social channels
    4. Via QR codes on your products, at in-person events, or on printed materials
