Form Builder: How To Create A Survey
You can :
- Gather feedback on your products.
- Provide your team with the information they need to best serve your customer. Add it to your post-purchase communications so shoppers can share their thoughts.
- Directly address retail buyers' questions/concerns with consumer data.
To Create A Custom Feedback Form:
- In your dashboard, select “Forms” from the side menu.
- Select the “Feedback” tab on the main page.
- Click the "+Create" button.
- Set up the form:
What kind of form would you like to create?: Select the "Feedback" option.
Form Subtitle: The small text below the Form Title.
Active: When toggled on (green), submissions will be accepted. If unchecked, no one will be able to submit the form. To remove the feedback form from your request form, simply set the form to “Deactivated”.
Show in consumer app: keep toggled on (green) so the form is active.
Sequential: when toggled on (green), questions will be presented to the viewer one at a time.
Prevent retaking: toggle on (green) to allow customers to only submit the form once.
Anonymous: toggle on (green) if you do not want to capture the respondent’s name and email address. These fields will not be displayed on the feedback form when the box is checked. Consumers that submit this form will not be added to your Audience.
Click Submit.
- Click the “Add Question” button:
- Under the Question Settings section, set up your question:
- Type: select the structure of the question you want to ask.
- Label: Type the question you want to ask.
- Placeholder Text: This is the text that appears in the response field before a response is typed.
- To add an additional question, click “Add Question” and complete the Question Text and Placeholder Text fields.
- Description: use this for any clarifying information you'd like to share.
- Additional Settings
- Required: check this box if you'd like the question to be required.
- Under the Question Settings section, set up your question:
- Click the "Ending" tab to customize the page displayed after a customer submits a form.
- Add a link to your website, a DTC discount code, or a warm thank you for participating.
- Click “Save” to preserve the form.
- To create a link to the survey to share with your audience, open the "Tracking links" section.
- Click the "Create link" button.
- Fill out the form:
- Title: this is the internal name for the link:
- Active: toggle on (green) to accept submissions to the form
- Fullscreen: toggle on (green) to display the survey as full screen without the WeStock App menu at the top.
- Add UTMs to track web traffic in analytics tools: toggle on (green) to add UTM parameters to understand where customers encounter the survey.
- Generate a QR Code to share anywhere people can scan it: toggle on (green) to create a downloadable QR code to add to marketing collateral, packaging, and in-store signage.
- Click "Submit" to create the link.
- The link will now be listed under "Tracking Links"
- Click the "Copy" button to copy the link to paste in your marketing
- note that for Click and Conversion data to track properly, the URL should be shared as written on this page, not the URL to the webpage the link opens.
- Click the "Copy" button to copy the link to paste in your marketing
- Share the form with your audience.
- To view aggregated responses:
- Click "Forms" on the left side menu.
- Select the form from the list of forms on the "Feedback" tab.
- To download all responses:
- Next to "Question Summary", click "View all responses"
- Click the "Download" button to export as a .csv.
